If you would like me to coordinate your mental health care with your primary care physician, psychologist, or other care provider, contact me, and I will send you a link to complete a Release of Information prior to making contact with those individuals.
If you are wanting me to release all or a portion of your file with Riverbend Therapeutics, contact me, and I will have you fill in a Release of Information prior to sending your file. I will request that you do this even if I am releasing my record to you as the client so I can keep track of where your records get released.
